Nuclear medicine is diversifying. Theranostics is expanding, gallium-based tracers such as PSMA-11 and DOTATOC are becoming part of routine practice, and clinical trials are multiplying. For departments, this growing range of radiopharmaceuticals is often expanding faster than staffing levels and available space.

CHRU de Nancy, together with Nancyclotep, has experienced this evolution. Between 2018 and 2025, its PET injections increased from 9,183 to 14,345. A fourth PET scanner was added, the radiopharmaceutical range more than tripled, from 4 to 17 available molecules (9 in routine use and 8 in clinical trials), and regulatory requirements became stricter. All of this within approximately 33 m² and with limited human resources.

The department redesigned its organization around automated dispensing. Two Unidose systems, operating in a through-wall dispensing, dispense the five most commonly used radiopharmaceuticals, including ⁶⁸Ga-labelled tracers. Each dose is prepared inside a Grade A shielded enclosure with 50 mm of lead shielding, with full traceability from prescription to injection. PET technologists collect doses themselves and manage their own patient workflow.

The results: 98% of dispensing activity automated, four PET scanners supported on a daily basis, and 50% of the second dispensing system’s capacity still available. The time saved is reinvested in patient care, maintenance and microbiological control.

“Producing at scale and in a continuous workflow, while maintaining optimal quality, with automated systems that improve with every update and a with automated systems that improve with every update and a pass-through channel that removes unnecessary handling. That’s the principle we rebuilt the department around, and today four PET cameras and around 15 radiopharmaceuticals run with one person in the hot lab.”

Dr Nicolas Veran, Radiopharmacist, CHRU de Nancy

As Dr Nicolas Veran, radiopharmacist at CHRU de Nancy, points out, the automated system does not achieve everything on its own: it is the organization rebuilt around it that delivers the results, allowing the team to focus its expertise where it truly matters.
